Our care managers are licensed in nursing, social work and physical rehabilitation, have decades of experience and are familiar with numerous health conditions such as dementia, heart failure, degenerative neurological, cardio-pulmonary disease, mental illness and end of life issues.

We help adults and seniors to experience normal aging and prevent crisis. We combine our passion for client advocacy, our knowledge of community resources and our ability to provide chronic care disease management to bring peace of mind to families and their medical providers.

Linda Smith, BSN, RN, CMC, CDP


Licensed registered nurse, certified Aging Life Care Professional™, certified dementia practitioner. 

Her experience with caring for adults and seniors spans more than three decades.

She uses the nursing process of assessment, critical thinking, and nursing diagnoses along with specific skill sets inherent

to the determination of treatment needs and respective costs, across the continuum of care. She provides coordination of patient care, construction of care plans and consultative services to families and professionals.

Linda discovered her talent as an entrepreneur and client advocate and established this boutique care management firm in 2009.

Her tireless capacity to develop creative solutions results in positive outcomes for her clients. She is knowledgeable on a wide range of medical, psychiatric, disability, geriatric conditions and community resources.

She blends these talents with advocacy and out of the box possibilities. "My goal is to help the client reach their maximum independence, function and adherence to treatment while maintaining client satisfaction and self-determination."

She strategizes with medical providers, social service agencies, case managers, estate attorneys, financial planners and other professionals in order to maximize services.

After receiving her bachelor of science in nursing from Simmons College in 1985, Linda gained a broad range of experience at Massachusetts General Hospital as a post operative surgical nurse, insurance quality and utilization review nurse at Blue Cross Insurance Co., then over 10 years nursing clients with complex medical and surgical conditions back to health along with assessment, evaluation, teaching, and medication management for Medicare certified home health agencies.

She is a member of The Aging Life Care Association, The Alzheimer's Association Partnership, The Mass Guardianship Association, the Brain Injury Association of Mass. and the Community Emergency Response Team (CERT) for the town of Wayland.

She and her team are continually learning and staying abreast of changes in the healthcare & homecare industry. They offer stimulating panel educational events for professional groups and families offering fresh perspectives regarding navigating the health care system, learning to be an effective family advocate and planning for successful aging.

Raised in Weston, MA she and her husband/business partner have raised two daughters in their 20's in Wayland. When she isn't writing care plans, you can find her indulging in her love of country dancing, yoga and color analysis. Proficient in Spanish. 

Seiko Hori, MS, RN, CDP

Nurse Geriatric Care Manager

A 1988 graduate of Temple University with a nursing degree as well as a masters in gerontology, Seiko has acute, long term care and community home care experience.

A native of Japan, she has a special interest in elders, dementia and care management for the Asian population. Seiko understands how to adapt to changing situations, is inspiring to her clients and does what is needed to improve a situation. Fluent in Japanese.

Meredith Gingrich, OTR/L

Occupational Therapist, Care Manager

Meredith is an experienced registered licensed occupational therapist. She has a strong passion for helping people reach their potential through highly-focused, goal-driven care management and rehab therapy.

She believes that listening to the patient and having fun is critical to developing creative, high-impact results.

"I'm passionate about my career; its not just a job," explains Meredith.

Meredith began her career at a skilled nursing facility, where she became a rehabilitation director at several facilities. She managed interdisciplinary rehabilitation teams and worked closely with nursing services.

Meredith owned a non-medical home health care agency which provided personal care and home making services to adults with disabilities and seniors.

She focuses on delivering exceptional rehabilitation advocacy while she and her husband are raising an adorable daughter. 

Deborah Smulian, LCSW., BC-


Social Worker, Care Manager

Deborah has over 40 years as a psychotherapist, hospital case manager at Lahey Clinic, dance therapist, cancer support group leader and more.

Raised in En​gland she moved to Israel as a teen and settled in Boston. Her wit, charm and introspection helps her clients find meaning in their lives. Fluent in Hebrew. 

Heather S. Oluwole COTA/L, ACE Certified Health Coach

Heather is a licensed, certified occupational therapy assistant and an American Council on Exercise (ACE) certified health coach. 

Her experience training and teaching exercise and nutrition spans more than two decades. Heather began her career in health and wellness as a personal trainer. 

She then received her Associates in Science in Occupational Therapy and worked with seniors in skilled nursing facilities and home care settings treating patients with a wide variety of medical and surgical issues, such as cardio-pulmonary, kidney disease, orthopedic surgeries, complex medical conditions, as well as neurological disorders and dementia. She discovered her passion for blending fitness with occupational therapy’s person-centered holistic approach to senior in-home training.

Heather has a comprehensive toolset that leads to exceptional results of improved health and wellness for her clients. She states, "I am passionate about helping adults and seniors reach their potential through highly-focused, goal-driven programs and my clients have more fun as they stay engaged with the activities they love and want to do."

Heather has lived in the Metro West Boston area for over twenty years. She and her husband raised two sons, now in their 20’s. She enjoys outdoor exercise, learning new hobbies with friends, volunteering in the community, gardening, reading, traveling and sampling new ethnic foods.

Robert Smith (Bob)

Director of Business Development

Bob, Linda's better half, develops new business relationships and establishes processes for operations. He brings meaningful insight and humor to serious family situations. He has witnessed his wife’s tireless devotion to her clients throughout their 30 year marriage.

Bob has developed a deep understanding of the care management, disability and aging space from a business, technical and operational standpoint. "We pride ourselves on our ability to design individualized solutions and see positive outcomes for a wide range of family needs.”

A life long career in sales and management, Bob’s insight and mediation talent helps families envision the possibilities.

Born and raised in Chestnut Hill, Newton, a graduate of Penn State, his numerous interests include cycling, cooking and relaxing in nature.
